What is Cornea Services?
Cornea services at ICARE Eye Hospital provides comprehensive management of diseases of the cornea and anterior segment.

Cornea is the front part of the eye ball which is transparent and watch glass shaped. It accounts for two thirds of the refractive power of the eye. Any disease process affecting the cornea makes it lose its transparency. This affects the vision of the eye.

Disease processes affecting the cornea are:
  • Microbial Infections
  • Congenital Snomalies
  • Corneal Dystrophies
  • Ectasias
  • Trauma
  • Chemical Injuries
Microbial Infections:
Bacterial, viral, fungal and parasitic infections of the cornea may be sight threatening and at times, serious enough to cause loss of the eye.

Timely diagnosis of the pathogen and appropriate treatment can help to save the eye and restore vision.

ICARE cornea department is supported by a microbiology laboratory for isolation of the organism.

Chemical Injuries:
Industrial and domestic use of chemicals is widespread. Many patients with acid and alkali injuries to the ocular surface are successfully treated at ICARE eye hospital. Early and appropriate medical treatment followed by surgical interventions in the form of amniotic membrane grafts and autologous stem cell transplants are undertaken at ICARE.

Support Systems for Cornea Services at ICARE
  1. Contact lens service
  2. Microbiology laboratory
  3. Eye bank
  4. Investigations
Investigations:
Specular Microscopy
A Specular Microscope helps to document the health of the corneal endothelium and diagnose early cases of Fuchs endothelial dystrophy. The appropriate management of cataracts with compromised corneas is also helped by specular microscopy. Keeping a track of the endothelial cell count pre and post penetrating keratoplasty is made possible with clinical specular microscopy. Specular microscopy is also important before collagen cross linking and phakic intraocular lens implantation.
Photoslit Lamp
Documentation of corneal pathology is aided by the Haag Streit slit lamp with Sony camera and a computerised record is maintained.
Tonopen
Intraocular pressure measurement in irregular, scarred, edematous corneas may be more accurately measured with this instrument. While applanation tonometry(widely available at ICARE eye hospital) is still the gold standard for intraocular pressure measurement , tonopen provides a quick and reliable measurement in scarred, irregular corneas and in post penetrating keratoplasty where there may be variable corneal thickness and astigmatism.
Corneal Topography
ICARE eye hospital has Orbscan II which can evaluate the topography of the cornea as well as measure the corneal thickness. The orbscan has placid and slit scanning technology which can assist in detecting forme frusta keratoconus, post lasik ectasias, posterior keratoconus. Planned selective suture removal in penetrating and anterior lamellar keratoplasty is made easy with this technology.
Ultrasonic Biomicroscopy (UBM)
UBM is an important tool in scarred and hazy corneas. Angle structures, crystalline lens, anterior or posterior chamber intraocular lenses can be visualized in cases where there is no visibility of the anterior segment.
Microbiology Laboratory:
Equipped with an Olympus self illuminating microscope. Corneal scraping in cases of microbial keratitis is done to accurately identify the pathogens. KOH, Gram, Geimsa, Acid fast staining (1 % and 20 %) is done routinely in all cases of microbial keratitis. Plating of saborauds dextrose agar and blood agar culture plates is also done routinely. All cases of microbial keratitis are documented and followed up till complete healing occurs.

Eye Bank:
Eye bank at ICARE eye hospital was established in 1997. It is registered with the eyebank association of India(EBAI) and with the Ministry of health govt of UP. ICARE eye bank is ably supported by the Sanjay Anand trust. It is well equipped with
  • Eye bank specular microscope
  • Refrigerator
  • Laminar flow hood
The cornea services of ICARE eye hospital with support of the eye bank has performed more than 600 keratoplasties over the last few years.
